Every appointment request is reviewed individually. Your visit is confirmed only after Karen approves the location, service, and time.
How to get started
1. Request a home visit. New patients should select Home Initial Eval. Returning patients may select Home Follow-Up. Provide the visit location so Karen can review availability.
2. Wait for confirmation. Karen reviews the request and confirms the location, service, and appointment time. A request is not a confirmed appointment.
3. Complete your secure intake forms. For an initial evaluation, complete the patient packet before your visit using the secure link provided with your confirmed appointment.
4. Meet with Karen at home. Your first visit includes an individualized evaluation to guide the care plan.
